Thank you for your time and assistance with the unannounced renewal inspection conducted on February 15, 2017. The on-site inspection was conducted from 10:20a.m. to 1:10p.m. On this date, there were 34 children in the care of four staff members in two classrooms. There were a total of 38 children enrolled in the preschool program. Four children's records were reviewed and one staff member's record was reviewed. Four staff members and two children were interviewed. The medication and medication paperwork was reviewed for one child. Lunch was served by the elementary school and was observed to meet USDA guidelines. All interactions between the teachers and the children were age appropriate and positive. Some of the items observed and reviewed included: documentation of emergency evacuation and shelter-in-place drills, required posted material, injury reports, current fire and health inspections, the first aid kit and emergency supplies, centers, circle time, language art activities, table top math activities, bathroom and hand washing procedures, transitions, lunch, and nap. Standard #: 22VAC40-185-90--A Description: Based on review of four children's records and verification from the staff, the center failed to obtain two of the three required parental agreements for one child. Evidence: 1. The record for Child #2 did not contain two agreements signed by the parent which state that if the child should become ill, the parent will pick the child up from the center as soon as possible and that the parent will inform the center within 24 hours if the child or any member of the household should develop a communicable disease. Date of enrollment for Child #2 was 11/14/2016. 2. Staff Member C verified that these two parental agreements had not been obtained. Plan of Correction: The staff will send home the center's Parent Agreement to Report a Communicable Disease form with the child to have the parent complete and return it. The Family Advocate will be contacted to ensure the center's Home School Partnership Agreement, which contains the other required agreement, is completed by the parent.
