Today we discussed risk ratings and the different types of licenses available at renewal.

A complaint was received on September 6th and initiated on September 12th regarding early morning ratios. A second complaint regarding ratios was received on 12/15/16 and initiated 12/21/16. The investigation was concluded on 2/9/17. During the investigation staff were interviewed, ratios were counted, and face-to-name sheets were reviewed. The information gathered during the investigation supports the allegation and complaint is valid. A violation regarding lack of implementation of ratios was sited and may be viewed on the violation notice.

Description: Based on observation and interview the center failed to ensure that when children are in regularly ongoing mixed age groups the staff-to-child ratio applicable to the youngest child in the group will apply to the entire group. Evidence: 1 - Staff 2 reports that during the early morning hours the center has been out of ratio in the opening classroom on multiple occasions. 2 - Staff 3 stated that in months past the opening classroom was routinely out of ratio 2 or 3 days per week. 3 - On 2/1/17 the face-to-name sheet shows at 7:20am there were 10 children present ages 2 through 9 years with one staff. 4 - Staff 1 confirmed that the room was out of ratio on 2/1/17 at 7:20am.

Plan of Correction: New director was in place effective January 3rd 2017. Schedule was reviewed and changed to reflect the early attendance of children. On 2/1/17 a child came in earlier than normal and immediately the opening teacher contacted the supervisor and coverage was maintained within several minutes. Going forward schedules are reviewed daily to properly ensure enough coverage. All staff will be retrained on how to manage staff to child ratios by 2/10/17. Management will ensure proper staff to child ratios by inspection several times per day. If a classroom is ever about to be over teachers need to immediately notify management so management can step in. DM will inspect on routine visits. This has been fixed since 2/1/2017 with systems in place.
