Comments:
An unannounced renewal inspection was completed on 2/16/17. There were 10 children present with two staff supervising. Two children's records and two staff records were reviewed. No medications at the site.

The inspector arrived at the center at 9:30 am and departed at 1:45 pm.

The children were observed playing with toys in the adjacent (special activity/fellowship hall) building, during a special activity (National Read Aloud Day) with parents, during outside time, during lunch, and during TV time.

The inspection was amended on 3/22/17 as per a follow-up inspection made on 3/13/17. This amended report was emailed to the licensee on 3/22/17 for signatures of the amendment.

Violations:
Standard #: 22VAC40-185-50-A
Description: Based on observation, the center failed to ensure that children's records were treated confidentially. Evidence: 1. There were two parent-teacher conference forms that were filled out with information related to one specific child's development observed in the classroom on a small toy shelf at the entrance door to the classroom. The forms were not in a folder or did not have anything covering them. The staff and the children were not in the classroom at arrival of the inspector and did not return to the classroom until 1:00 pm. There was a parent activity scheduled on the date of the inspection in which parents were observed by the inspector to come into the classroom without knowledge of the staff.

Plan of Correction: This was corrected during the inspection. The teacher gathered the forms and placed them in a folder and took them into the building where the special parent activity (and where staff was present) was being held. In the future, the staff will keep records in a secure place.

Standard #: 22VAC40-185-320-B
Description: REPEAT VIOLATION

Based on observations, the center failed to ensure that each rest room area provided for children met the requirements of the Standards regarding the location and the temperature of the water.

Evidence:
1. The hot water was observed in the sinks of the two bathrooms used by the children and the temperature was measured with a digital thermometer at 11:00 am. The temperature of the water was 129 degrees Fahrenheit in one sink and 127.5 degrees Fahrenheit in another sink. The temperature is required to be maintained below 120 degrees Fahrenheit.

Plan of Correction: The teacher will report the hot water to the site manager and the director. The teaching staff will check the temperature of the water weekly to ensure it is maintained below 120 degrees.

Standard #: 22VAC40-185-550-F
Description: Based on observation, the center failed to ensure that the required emergency numbers were posted in a visible place at each telephone. Evidence: 1. There was a cordless handset and base observed in the storage room of the modular unit. There were no emergency numbers posted at the telephone. The handset was observed being taken to the adjacent building to be used while the children and staff were in that building for a special activity and lunch. The children and staff were not in the modular unit until 1:00 pm during the inspection. 911 and poison control numbers are required to be at each telephone.

Plan of Correction: This was corrected during this inspection. Both emergency numbers were posted on the handset of the cordless phone. Staff will ensure that emergency numbers are posted at each telephone for future.
