Comments:
An unannounced renewal inspection was conducted on Wednesday, February 22, 2017, from approximately 9:15am until 12:15pm. The following staff to child ratios were observed: Room 1 (2's) seven children with two staff Room 2 (Pre-K) 15 children with two staff Room 3 (3's) nine children with two staff Room 4 (3's) ten children with two staff Room 6 (3's) eight children with two staff Room 7 (Pre-K) sixteen children with two staff Activities observed today included children completing puzzles, constructing with blocks and legos, practicing letter tracing in sand, discussing and practicing tooth brushing, creating easel art designs with purple paint and discussing books. The children also enjoyed gross motor play both outdoors and in the atrium. A practice shelter-in-place and a practice evacuation drill was conducted 1/6/17. Four children's records, four staff records and medications for two children were inspected.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on inspection of the facility, the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The following items were discussed with management: 1. A ceiling vent in Room 6 is not secure and could fall . 2. An outdoor playground unit has a chain/rope that is covered with plastic that has become brittle and sharp exposing rusted chains. 3. There are several ceiling tiles that are stained and exchange vents that are coated with dust.

Plan of Correction: Per Center: "Vent in room 6 will be fixed. Exchange vents will be cleaned. Cleaning staff will be alerted to these concerns so that this does not happen again. Outdoor playground equipment - chain/rope will be removed or repaired."

Standard #: 22VAC40-185-280-B
Description: Based on inspection of the facility, the center did not ensure all cleaning materials were kept in a locked place. Evidence: There were unlocked cleaning products stored on a high shelf in the bathroom in the two year old classroom. There were unlocked cleaning products located on top of a cabinet in the main hall bathroom.

Plan of Correction: Per Center: "Cleaners were immediately removed. Director will retrain staff on the importance of locking up cleaning materials. Concerns will also be addressed with custodial staff. "

Standard #: 22VAC40-185-350-E-4
Description: Based on inspection of the facility, the center did not ensure staff to child ratios in the four year old classroom. Evidence: The staff to child ratio for this age group is one staff person for every ten children. For approximately ten minutes, a staff person was left alone to supervise fifteen children.

Plan of Correction: Per Center: "A second staff was immediately brought into the room when this was brought to directors attention. A plan was put in place to ensure coverage during music time on Wednesdays."
