Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 1:10 PM and departed at 3:20 PM. Eighty children were in care with ten staff. Nap time, afternoon snack and bus departure were observed. A sample size of five children's records and three staff records were reviewed today. Medication administration was reviewed. No children's injuries were reported. An exit meeting was conducted with the program director during which the facility's plan of correction was obtained. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based upon review of three staff records, the facility has not ensured that a tuberculosis screening is obtained for staff at least every two years. Evidence: According to the record provided for staff 2, the most recent tuberculosis screening was obtained 08/18/2014.

Plan of Correction: The facility responded with the following: All staff records will be checked to ensure tuberculosis screenings are obtained every two years.

Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records, the facility has not ensured that each child's record includes the names, address and telephone numbers for two designated persons other than the parents to be contacted in an emergency when a parent cannot be contacted. Evidence: The record for child 5 does not have the street address for one of the two designated persons to be contacted in an emergency when a parent cannot be contacted.

Plan of Correction: The facility responded with the following: The parent will be contacted to obtain the address for the emergency contact person. Children's records will be checked to ensure records are complete.

Standard #: 22VAC40-191-40-D-1-C
Description: Based upon review of three staff records, the facility has not ensured that they obtain a child protective services (CPS) central registry finding before three years since the dates of the last CPS central registry finding. Evidence: The most recent CPS central registry finding in the record provided for staff 1 is dated 10/16/2013.

Plan of Correction: The facility responded with the following: A CPS central registry finding will be obtained for staff 1.
