Staffing records are reviewed at the main office on a separate date.
An unannounced monitoring inspector was conducted March 1 2017 between 7:30am and 8:35am. There were 26 children in the direct care of 4 staff members. Children were observed building, drawing, playing various gym games, and playing board games. A sampling of children's records, medications, the evacuation drills, medication training, the physical space, and supervision were observed and inspected. Standard #: 22VAC40-185-60-A Description: Based on record review, 1 of 3 records did not contain the required information. Evidence: Child #1 record did not contain a 2nd emergency contact, including name, telephone number, and address. Plan of Correction: We will talk to parents and will get another contact.
Standard #: 22VAC40-185-420-E-3 Description: Based on record review, 2 of 3 children's record did not contain documentation that a parent reviewed the record at least annually. Evidence: Child #2 last documented review was dated 9/3/2015 and Child #3 last documented review was dated 9/15/2015 Plan of Correction: Will go through each file and make sure parents review the record.
