Comments:
An unannounced monitoring inspection was conducted beginning at 10:45 and ending at 2:15. There were 53 children in care, ages 2 months through 5 years of age. An additional group of school age children will arrive after school. All classrooms were in appropriate ratio. Children were observed at outdoor play, doing educational activities, having snack of cereal bars and juice, cleaning up and having lunch of pigs in a blanket, mixed vegetables, milk and applesauce . Also observed naptime. Cot placement was appropriate in all classrooms. Reviewed medications for 3 children - see violation notice Reviewed 4 new staff file s- complete Reviewed files for 5 children - see violation notice Emergency escape drills and shelter in place drills are up to date. Violations:
Standard #: 22VAC40-185-150-B
Description: Based on a review of 5 children's files, it was determined that 1 child did not have a shot record that was signed by the physician. Evidence: The shot record for child #5 was a computer print out and was not signed or stamped by the Doctor/office

Plan of Correction: The staff will assure that the form is signed by the doctor or stamped by his office.

Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that not all areas of the center are maintained in a safe condition. Evidence: The doors on the tall cabinets are not able to be secured in two classrooms, the two's and the threes. The Three's cabinet is completely missing a door.

Plan of Correction: The Director has contacted the president of the building and the doors will be fixed within two weeks.

Standard #: 22VAC40-185-510-D
Description: Based on a review of the medications for 3 children, it was determined that one child had a permission form that was expired. Evidence: The medication for Child #5 has a 10 day form signed by the parent. The form expired in January. A long term form signed by the doctor is needed or else the parent will need to redo the short term form every 10 days.

Plan of Correction: The Staff will notify the parent today for a new form.

Standard #: 22VAC40-185-510-E
Description: Based on a review of medications for 3 children, it was determined that the medication for one child did not have a prescription label. Evidence: The emergency medication for Child #6 did not have a box with a prescription label.

Plan of Correction: Staff will notify the parent that the prescription label must accompany the medication.
