Discussed with Director: Transition to the outside for all groups today was not a smooth transition.

An unannounced monitoring inspection was conducted today. All rooms were in ratio during the inspection. 4 staff and 5 children's files were reviewed. See violation notice form missing information. Children were observed eating lunch , during diaper changes, hand washing, and "transitioning" to outside. If you have ay questions regarding this inspection, please contact me at 703-479-4678. Standard #: 22VAC40-185-70-A
Description: Based upon a review of files, staff files are incomplete. Evidence: 3 of 4 files reviewed did not contain documentation of orientation. (Staff 2,3,4)

Plan of Correction: Orientation was conducted, just not documented well in the file. We will henceforth use a different form that documents the required components.

Standard #: 22VAC40-185-420-E-1
Description: Based upon staff statements, tummy time is not being documented for infants that require 30 minutes a day.

Plan of Correction: Staff will utilize the already existing tummy time record in the online application.

Standard #: 22VAC40-185-510-C
Description: Based upon a review of medications, one medication authorization is expired. Evidence: Child A's authorization for Children's Ibuprofin expired 1/31/17.

Plan of Correction: The medication will be sent home.

Standard #: 22VAC40-191-40-D-1-B
Description: Based upon a review if files, one staff file is incomplete. Evidence: One staff file did not contain documentation of a completed Sworn Disclosure Statement, prior to the first day of employment. Evidence: Staff #4 did not have a completed Sworn Disclosure Statement on file. (Date of Hire 1/19/17)

Plan of Correction: The form will be completed today. The staff stated he did completed the form; however, it must have been misfiled.

Standard #: 22VAC40-191-60-C-1
Description: Based upon a review of staff files, one staff member was allowed to work without a completed Sworn Disclosure Statement on file prior to the first day of employment.

Plan of Correction: Staff will not be allowed to work without the completed form on file.
