- Technical Assistance:
1. Discussed challenges of including children with special needs into the program, and behavior management.
Conducted an unannounced renewal inspection 3:10-5:40pm. Observed 98 children + 15 direct-care staff. Ratios were in compliance, but at times supervising was lacking. Children were observed doing homework, washing hands, eating snack, doing art, having outdoor active-play, and departing for the day. Staff were talking and playing with the children, and greeting parents as they arrived. Head counting was observed. Snack served: yogurt, pears, and water. Equipment and supplies were sufficient. Medication management was in order. Emergency drills completed and written injury reports well documented. Health and fire inspections were current. Standard #: 22VAC40-185-60-A Description: Based on records reviewed, it was determined that 1 of 3 children's records did not include required information. Evidence: The enrollment paperwork for Child #1 indicated that the child has an IEP however, documentation of such was not on-file. Plan of Correction: Site Director will obtained a copy of IEP from the parent. [sic]
Standard #: 22VAC40-185-270-A Description: Based on observations made, it was determined that an area and a piece of equipment in the center were not maintained in a clean, safe and operable condition. Evidence: 1. The foosball table located on the stage was missing a leg. 2. The carpeted wall divider in the stage area was stained and dirty with black/brown substances. In addition to this being a wall, the children's bins and belongings were pushed up against this surface. Plan of Correction: Even though foosball table was not out for the children to play with site director will coordinat with building engineer for proper disposal [sic] Site director will communicate with building engineering to address the stains on the sliding partition located on the stage.
Standard #: 22VAC40-185-340-A Description: Based on observations made, it was determined that staff did not ensure the care, protection, and guidance of two children present in the cafeteria. Evidence: 1. Child #1 and Child #3 were observed climbing up the shelves in a storage cabinet (located on the stage) like a ladder. They proceeded to climb to the top shelf and pulled a flashlight out of the cabinet and a storage bin containing a paper hole punch and scissors. 2. There was some confusion as to who was responsible for supervising these children at the time, as the Site Director identified Staff #2 as being responsible, while Staff #2 said it was a shared responsibility among staff present. Staff #2 had stepped out of the cafeteria and into the homework room for a minute or less when the children were observed climbing the shelves. 3. Child #1 and Child #3 were observed running around the cafeteria and stage area the entire inspection. While running they were observed getting into and throwing activity supplies. Additionally, Child #1 was observed taking napkins from the snack area, standing on furniture, sucking on a balloon, playing with paper trash, and opening/closing windows. On one occasions Child #1 was observed taking a pack of art paper from the staff storage closet and running with it onto the stage area. 4. Although areas of the cafeteria were set up for reading, blocks, and drawing for all children, there were no organized activities for these children. Periodically these children engaged briefly in the free-play activities available. 5. On one occasion the inspector alerted staff that Child #1 and Child #3 were physically fighting, which resulted in Child #3 being hit in the face and head. 6. Staff stated that due to their special needs it was difficult for the children to remain with their assigned age group, and as a result they stay in the cafeteria where they are supervised by whomever is present. There was minimal to no information on-file describing the needs of the children, ways the staff could successfully integrate the children into the program, or how to manage behaviors. 7. There were no staff assigned to provide activities for these children, as all staff present were assigned to other groups, and/or responsible for other activities and tasks. Plan of Correction: Site director will cintinue to coordinate with director of operations regarding screening, and interviewing potential staff for the program. All staff will properly supervise all children in each area to ensure that they are being cared for, protected, and properly guided. Site director will provide additional training in behavior management to staff in order to ensure the safety of each child in enrolled in the program. [sic]
