Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, March 07, 2017 from 10:45am to 1:00pm. The children were observed playing outside on the playground. The infants were observed playing on the floor with their teachers. All classrooms and playgrounds were inspected today. The center's mini buses were inspected as well. Four children?s records and seven employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/28/2017 Last shelter-in-place drill: 3/01/2017 Today, the following child to staff ratios were observed: Infants ?7:2 Toddlers-6:2 Twos ? 16:3 Threes ?16:2 Pre-K ?10:1 Junior K ?3:1 School Age ?not present The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector, Roshon Casey, at (804) 662-9760. PROVIDER: Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation on 03/07/2017, the center did not ensure that where playground equipment is provided the resilient surfacing complied with minimum safety standards. EVIDENCE: The pea gravel on the preschool/ pre-k,playground with the sliding equipment, and the school age playground with the swings measured less than the minimum required depth of 6 inches.

Description: Based on observation on 03/07/2017, the center did not ensure that the required first aid kits on the buses contained the required items. EVIDENCE 1. The thermometer in the first aid kit on Bus #1 did not work. 2. The first aid kit on Bus #2 did not contain band-aids.

Description: Based on a review of staff records on 03/07/2017, the center did not ensure that three of the seven records reviewed contained a a central registry finding within 30 days of employment. EVIDENCE: 1. The central registry finding in the record of Staff #1, whose date of hire is 11/13/2016, is dated 1/6/2017. 2. The record of Staff #5, whose date of hire is 11/07/2016, did not contain the results of the central registry finding. 3. The central registry finding in the record of Staff #6, whose date of hire is 11/07/2016, is dated 1/12/2017.
