Standard #: 22VAC40-185-550-M
Description: Based on a documentation review, the licensee did not ensure the center maintained a written record with all required information of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: During the inspection on March 8, 2017, 10 injury report records were reviewed and 2 did not include the time the parent was notified of the injury and 7 did not include how the parent was notified of the injury.

Plan of Correction: Management has put systems in place as of 3/13/17 that will ensure that all incident reports have all required notations complete prior to parent signature's. DM will review quarterly for compliance.
