Violations:
Standard #: 22VAC40-185-550-L
Description: Based on record review of injury reports, the center does not have documentation to verify that parents were notified of accidents. The Inspector reviewed October's accident reports and although forms are signed by one employee that parent was called with the time of contact, it does not include the parent's signature or a second staff member's signature.

Plan of Correction: We have corrected and will make sure there are two signatures- either the parent's or the second staff member's signature.
