Technical Assistance:
Discussion was held on the topics of current enrollment and current staffing.

The following inspection took place between the hours of 10:30 am through 1 pm. Today, I met with the Staff of the Chesterbrook Academy #822 located in Fair Lakes, Virginia. At the start of this unannounced monitoring inspection there were a total of 93 children with 11 staff; good ratios in all 6 rooms. A total of 5 children?s files and 3 new staff files were reviewed. The children were engaged in the following: games, art, theme of St. Patrick's Day, song, dance, diaper changing, bathroom breaks, hand washing, story time, center within the classroom, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-130-B
Description: Based on review, 1 of 5 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/17/2017), the most current immunization documentation for Child #5 was dated - 07/18/2016. 2. Based on the information contain within their record, on the date of inspection, Child #5 was under the age of two years.

Plan of Correction: An update immunization page shall be obtained.

Standard #: 22VAC40-185-270-A
Description: Based on observation, there were a few areas inside the center that were not maintained in a safe condition. Evidence - 1. On the date of inspection (03/17/2017), the following areas were observed to be in non-compliance: a. In the Pre-K1 room there was peeling paint under a classroom window next to the door to go outside. b. In the Intermediate room there chipped paint on the window sill, a vent/wall grate near the cots that was not flushed with the wall and could allow children's fingers access behind it, and the door to the outside has its weather stripping come partially off. 2. All listed areas were accessible to the children in care.

Plan of Correction: Maintenance shall be contacted in order to make the appropriate fixes.

Standard #: 22VAC40-185-550-E
Description: Based on review, the center did not maintain a record of the dates of the practice drills for one year. Evidence - 1. On the date of inspection (03/17/2017), drills conducted between March 2016 through November 2016 were not available for review. 2. This was confirmed on the date of inspection by the Center Director.

Plan of Correction: Drills shall be maintained and recorded within the center log. Effort shall be made to locate this past log sheet.
