22VAC140-185-450 B Linens shall be clean and washed at least weekly.

Comments:
An unannounced monitoring inspection was conducted on March 22, 2017. Upon arrival to the facility, there were 40 children with seven staff members. Additional staff were on site for administrative and support purposes. The center has a total enrollment of 98 children at this time. Children were observed having morning snack, coloring, reading books, playing freely and during outdoor play. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. Three medications were reviewed. Three children's records were reviewed. Four staff records were reviewed. The following playground equipment is currently not in use: the large wooden ship, the swings at the rear of the building and Merry Go Cycle. Exit interview was conducted with the Program Supervisor.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, the licensee did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on March 22, 2017, there was no documentation of a TB screening in the record for Staff #2 (start date 2/27/17).

Plan of Correction: This staff member has an appointment for Friday of this week.

Standard #: 22VAC40-185-40-J
Description: Based on review, the licensee did not ensure that injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: During the inspection conducted on March 22, 2017, the most recent review of the center's injury prevention procedures was dated 10/01/15.

Plan of Correction: We will write up a new one and complete it annually with our director.

Standard #: 22VAC40-185-70-A
Description: Based on record review, three of four staff records reviewed, the licensee did not ensure that each staff person's record was documented with all required information. Evidence: 1. During the inspection conducted on March 22, 2017, in the record for Staff #1 and Staff #2 there was not a signature on one of two reference checks conducted over the phone. 2. In the record for Staff #3 there was not an address for one person to contact in the event of an emergency and there were no signatures of two reference checks conducted over the phone.

Plan of Correction: All records were updated and corrected during the inspection.

Standard #: 22VAC40-185-240-D-4
Description: Based on observation and review, the licensee did not ensure that any child for whom emergency medications (such as but not limited to albuterol, glucagon, and epipen) have been prescribed shall always be in the care of a staff member or independent contractor who meets the requirements in subdivision 1 of this subsection (Medication Authorization Training) Evidence: During the inspection conducted on March 22, 2017, two children who have been prescribed emergency medications arrived at the center at approximately 8:20am and 8:40am. A staff member with MAT training did not arrive to the facility until approximately 9:00am.

Plan of Correction: We will get some opening and closing staff certified in MAT training. Staff person with current MAT training will get updated training and Program Supervisor will also be scheduled for the training.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. During the inspection conducted on March 22, 2017, in the Learning Lab, the licensing inspector observed the following items in an unlocked cabinet under the sink: a bottle of disinfectant, a bottle labeled sanitizer, antibacterial hand soap, a bottle of bleach and a container of powdered gel absorbent. 2. All of these items were labeled keep out of reach of children and warning or caution.

Plan of Correction: The items were locked during the inspection.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation and inspection of the facility, the licensee did not ensure that ratios were maintained for children from birth to the age of 16 months: one staff member for every four children. Evidence: During the inspection conducted on March 22, 2017, the licensing inspector observed a staff member taking a scheduled break at 12:00pm leaving one staff person with eight children during naptime. The youngest child in the room was 13 months old. A second staff person arrived in the room at approximately 12:10pm.

Plan of Correction: The ratio was corrected during the inspection.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and inspection of the facility, the licensee did not ensure that ratios were maintained for children from three years to the age of eligibility to attend public school, five years by September 30: one staff member for every 10 children. Evidence: During the inspection conducted on March 22, 2017, the licensing inspector observed one staff member with 11 children ages three years old-five years old.

Plan of Correction: The ratio was corrected during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that the diapering surface shall be used only for diapering or cleaning children, and it shall be cleaned with soap and at least room temperature water and sanitized after each use. Evidence: 1. During the inspection conducted on March 22, 2017, in the Woodchucks classroom, the licensing inspector observed a bead maze toy and a box of tissues on the diapering pad. 2. In the Infant room, the licensing inspector observed a staff member changing a child's diaper. The staff member sprayed the surface with a sanitizing agent and immediately wiped the surface with a paper towel. The staff member was then observed resting a clipboard on the diapering pad while updating the Infant Daily log. 3. In the Chipmunks room, the licensing inspector observed a child's diaper being changed. Children were observed playing on the diapering surface after the child was changed and the surface had not been sanitized.

Plan of Correction: Items were removed during the inspection in the Woodchucks classroom. I will go over with staff the correct way to wash their mats and to have their diaper changing procedure by the changing table.

Standard #: 22VAC40-185-560-F
Description: Based on observation and inspection of the facility, the licensee did not ensure that when centers choose to provide meals or snacks, the following shall apply: Centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). Evidence: During the inspection conducted on March 22, 2017, the licensing inspector observed the children having morning snack which consisted of granola bars and water. For snack centers should select two of four of the following food components: milk, fruit/vegetable, grains/bread and meat/meat alternate.

Plan of Correction: Inform kitchen staff and offer two food items during snack.

Standard #: 63.2-1720-F
Description: Based on observation and record review, the licensee did not ensure that no no employee shall be permitted to work in a position that involves direct contact with a person or child receiving services until an original criminal record clearance or original criminal history record has been received, unless such person works under the direct supervision of another employee for whom a background check has been received. Evidence: 1. During the inspection conducted on March 22, 2017, the licensing inspector observed Staff #2 (hire date 2/27/17) working alone with a group of children. There were no results of a criminal record clearance and this staff member was not in the direct supervision of another employee for whom a background check had been completed. 2. Staff #3 (hire date 2/27/17) was observed alone with a group of children during naptime when another staff member went on a scheduled break at 12:00pm. There were no results of a criminal record clearance and this staff member was not in the direct supervision of another employee for whom a background check had been completed until approximately 12:10pm when another staff person arrived with a completed check.

Plan of Correction: I will keep them in supervision of a staff member with a criminal background check.
