Comments:
An unannounced renewal inspection was conducted today, March 22, 2017 to determine the center's compliance with licensing standards. The inspector arrived at 10:00 am and concluded the inspection at 1:00 pm. Upon the inspector's arrival, children were engaged in a variety of activities. Pre-k children were observed playing in centers (trains/blocks, house keeping, doll house, and pattern cards & pegs), working one on one with staff reviewing letters and practicing writing skills (printing name and letters) and flower print painting for art. The preschool children had circle time where they reviewed colors, sang the name song and listened to a story. Toddlers were observed playing with blocks, trucks and musical instruments. Staff sat with children and interacted in a positive manner. Infants were held, fed and played on the floor with toys. Later lunch time and nap were observed. School age children were not in attendance during the inspection. All areas of the facility were inspected to include classrooms, kitchen and play ground areas. The facility is decorated with children's artwork. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and emergency supplies were inspected and found complete for the building and van used for transportation. five employee records and five children's records were reviewed during today's inspection. The emergency preparedness plan was current. Emergency medications and authorizations were reviewed. Two staff have current EMAT certification and Rectal medication certification. Health Inspection: 02/21/2017 Fire Inspection: 12/07/2016 The last documented evacuation drill was held on 03/21/2017. The two required shelter-in-place drills for 2017 were conducted on on 02/27/2017 and 03/21/2017. Today the following ratios were observed: Pre-K: ( 4 and 5 year old) 12 children with 2 staff Older Infants & Toddler: 11 toddlers with 3 staff. Preschool: (2 and 3 year old) 11 children with 2 staff. Infants: 7 infants with 2 staff. School age children: not in attendance. The monthly menu was posted and today's lunch consisted of: spaghetti, carrots, apples, roll and milk. The violations from the previous inspection on December 19, 2016 were checked for corrections. There was one repeat violation from within the licensing period. A complete renewal application has been received by the Department. Description: Based on observations, the facility did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: The seat cushion of the rocking chair in the Infant room is torn and exposes the interior foam posing a chocking risk.

Plan of Correction: Per the Owner: We will remove exposed foam cushion on rocking chair today. We will replace cushions on rocking chair.
