Comments:
An unannounced inspection was conducted on 3/1/17 for a self reported incident regarding the care and supervision of the children at the facility that was received on 2/27/17. Interviews were conducted and documentation was reviewed. Violations related to this self reported incident are listed on the violation notice attached to this summary.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on information obtained during the investigation, it was determined that the licensee has not ensured that the center's activities and services were maintained in compliance with the standards and the center's own policies and procedures. Evidence: 1. During interviews staff confirmed that they did not count the children that had been outside as they are required to do. 2. The facility's playground safety plan states that attendance is taken prior to re-entering the building. The staff did not follow this plan.

Plan of Correction: The facility repsonded: The center has purchased 2 way radios. The 2 way radios will be taken outside, by lead teachers, when each classroom exits the building to the playground. When they enter back into the building they will notify the office by the 2 way radio and the administrative (Center Director or Assistant Center Director) will step outside to make sure all children are in the building and also count off the children who have returned back into the building using the daily master attendance form. Effective March 16, 2017.

Standard #: 22VAC40-185-340-A
Description: Based on information obtained during the investigation, it was determined that the facility did not ensure that when staff are supervising children, they shall always ensure their care protection and guidance. Evidence: 1. The Licensing Inspector watched a facility provided video with staff #3 (Assistant Director) which showed that at approximately 11:00am child #1 was seen on the playground unattended after staff #1 and staff #2 entered the facility with 13 children from the Pre-Kindergarten classroom. Child #1 can be seen running around on the playground and then ringing the door bell to the facility. When no one in the facility answered the door, child #1 can be seen continuing to run around the playground. 2. A member of the community who lives adjacent to the center noticed the child and made contact with him to ask if he was okay when she did not see any staff. She then proceeded to enter the fenced playground area and escorted the child to the front door, where she rang the door bell and knocked on the door. 3. At approximately 11:02am, staff #3 (Assistant Director) opened the door and returned the child back to the Pre-Kindergarten classroom. 4. During interviews, staff #1 and staff #2 confirmed that neither of them were aware that the child was left on the playground and was not in the classroom.

When they enter back into the building they will notify the office by the 2 way radio and the administrative staff (Center Director or Assistant Center Director) will step outside to make sure all children are in the building and also count off the children who have returned back into the building using the daily master attendance form. Standard #: 22VAC40-185-340-F
Description: Based on information obtained during the investigation, it was determined that the facility did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff Evidence: 1. On 2/27/17, at approximately 11:00am, child #1 was left unattended on the playground for approximately two minutes. 2. Staff #1 and staff #2 were outside on the playground with a group of 14 children ages two to four years old. 3. The Licensing Inspector watched a video with staff #3 (Assistant Director) which showed at approximately 11:00am, staff #1 and staff #2 entered the facility with only 13 children and closed the door. 4. Staff #1 and staff #2 both confirmed that they had 14 children on the playground with them and did not realize that child #1 did not come into the facility from the playground with the rest of the children.

Plan of Correction: The facility responded: The center has purchased 2 way radios. The 2 way radios will be taken outside, by lead teachers, when each classroom exits the building to the playground. When they enter back into the building they will notify the office by the 2 way radio and the administrative (Center Director or Assistant Center Director) will step outside to make sure all children are in the building and also count off the children who have returned back into the building using the daily master attendance form.
