Technical Assistance:
22VAC40-185-70 A-1 Name, address, verification of age requirement, job title, and date of employment or volunteering; and name, address and telephone number of a person to be notified in an emergency shall be kept AT THE CENTER for each staff person present.

Comments:
An unannounced monitoring inspection was conducted on March 23, 2017. There were 12 children present with two staff members. The posted menu indicated that for lunch, the children would be served baked fish with cheese on a bun, cole slaw, oven baked fries, apricots and milk. Observations included indoor and outdoor areas, nutrition, supervision, staff/child ratios, health and safety, and the children's records. The children were observed playing in centers and working with staff on various activities. Three children's records were reviewed. There are currently no medications present in the center at this time.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of three children's records reviewed, the licensee did not maintain and keep at the center a record for each child which contained all required information. Evidence: 1. During the inspection conducted on March 23, 2017, the record for Child #1 did not include health information as required by 22 VAC40-185-130 through 22 VAC40-185-150 (Physical examinations for children). 2. The record for Child #2 did not include documentation of the work phone number for one parent.

Plan of Correction: 1. The child's physical was faxed during the inspection and added to the record. 2. We will talk to the parent and get the information.

Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on March 23, 2017, the most recent fire inspection for the facility was more than a year old and was dated 9/28/15.

Plan of Correction: Representative called the fire official's office and scheduled an appointment for tomorrow.
