- Technical Assistance:
We discussed the requirement to adhere to all regulations in the following: Standards for Licensed Child Day Centers, General Procedures And Information For Licensure, Background Checks For Child Welfare Agencies and the Code of Virginia. The owner was given copies of all of these documents as well as the related technical assistance documents and the Publication Order Form to order additional documents as needed. Hand washing requirements related to snack and diapering/toileting were reviewed. Hazardous substance storage was reviewed: although the half-door locks, items are accessible if stored on the open shelving. Children should have opportunities to participate in a variety of activities that are age and stage appropriate
An unannounced monitoring inspection was conducted beginning at 8:20 a.m. and ending at 10:05 a.m. with the director and owner. There were twenty-four children in care ranging in age from five months to five years of age with six staff. Infants were observed sleeping, in high chairs and bouncers. The toddlers and two year old classes were observed in the two year old class. Preschoolers were observed watching the movie "Trolls". Morning snack was observed and was oatmeal creme pies and apple juice. Three children's records and three staff records were reviewed. Standard #: 22VAC40-185-140-A Description: Based on review of documentation, the center failed to obtain documentation of a physical within thirty days of attendance. Evidence: Child A, first day of attendance 1/17/2017, did not have a physical in the record. Plan of Correction: The information will be requested from the parent at pick up today.
Standard #: 22VAC40-185-60-A Description: Based on review of documentation, not all children's records were complete. Evidence: Child A, first day of attendance 1/17/2017, did not have full addresses for two emergency contacts. Plan of Correction: The information will be requested from the parent at pick up today.
Standard #: 22VAC40-185-240-D-5 Description: Based on review of documentation, the center failed to obtain documentation of daily health observation training. Evidence: There was no documentation that at least one staff member on duty had obtained daily health observation training within the last three years. Plan of Correction: The course has been completed, documentation will be picked up on Saturday.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center were maintained in a safe, operable condition. Evidence: In the basement childcare area, there were two bottles of detergent and a box of dryer sheets stored on top of the hot water heater creating a fire hazard. There was a vacuum cleaner with the cord extended that created a tripping hazard. There is a loose plexiglass window pane on the right side of the room that allows cold air and debris such as leaves inside. In the main childcare bathroom, there is a broken toilet paper holder that has a sharp metal fastener exposed creating a scratch hazard. The door to the four's classroom has broken wood paneling that creates a scratch hazard. Plan of Correction: The vacuum and detergent were locked in the downstairs closet. Our maintenance guy will fix the window and door. We will fix the toilet paper holder or replace it.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all electrical outlets were equipped with protective covers. Evidence: In the toddler room, there were two outlets in an extension cord not covered. Plan of Correction: Removed during the inspection
Standard #: 22VAC40-185-430-B Description: Based on observation, there were not an adequate supply of materials and equipment for the Two's classroom. Evidence: There were seven children present with two teachers. There were plastic animals observed accessible to children in care. Plan of Correction: Toys will be brought up from downstairs and supplies will be labeled.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of documentation, the center failed to obtain satisfactory central registry finding within thirty days of employment. Evidence: Staff #1, start date 4/18/2016, central registry background check was dated 6/15/2016. Plan of Correction: In the future, background checks will be sent within the first week of employment.
