Technical Assistance:
Subsidy requirements were reviewed and documents were left with administration relating to these requirements. The subsidy inspection will be conducted at the next unannounced inspection as required.

Comments:
The licensing inspector conducted an unannounced non-mandated focused monitoring inspection, at which rest time was observed along with infant activities. Children were observed; and staff and children files and other documentation were reviewed. Twenty-one (21) children were found to be in care with 5 staff, and administration. Previous violations were reviewed, and two (2) of the violations from the previous inspection had not been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 12:20pm and concluded at 2:15pm.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff records and discussions with administration the facility failed to maintain compliance with this standard as required relating to TB tests. Non-compliance with this standard was previously cited on 11/29/16 and again today 3/22/17. Therefore this is marked as a repeat violation. Evidence: Staff record #4 did not have a renewed TB test as required. The TB test on file was dated 7/30/14. TB tests are to be renewed every 2 years.

Plan of Correction: Will request staff to obtain a new TB test.

Standard #: 22VAC40-185-60-A
Description: Based on review of children records the facility failed to maintain compliance with this standard as required relating to children allergies and chronic health problems. Evidence: Child records #1 and #2 did not address any allergies or chronic health problems as required. Child #1 enrollment date was 3/16/17. Child #2 enrollment date was 2/13/16.

Plan of Correction: Will review record with parents upon pick up today to complete. File review of all childrens records completed next week. In the future we will also review all new enrollee records with parents prior to attendance.

Standard #: 22VAC40-185-240-C
Description: Based on review of staff files and discussions with administration the facility failed to maintain compliance with this standard as required relating to annual training. Non-compliance with this standard was cited on 11/29/16 and again today 3/22/17. Therefore this is marked as a repeat violation. Evidence: Staff had not obtained any more training hours towards the required 16 hours of annual training to date.

Plan of Correction: We are currently working towards all staff having the 16 hours per year trainings.
