An unannounced monitoring inspection was conducted in the Center on March 20, 2017. There were 14 children and 2 staff present. Currently, 18 children are enrolled in the program. The children's ages range from 3 to 5 years old. The children were observed in their classroom. The classroom was equipped with age appropriate materials for the children's use. Staff were observed interacting with the children and provided guidance when needed. The children were also observed during the lunch meal in the school cafeteria. The children returned to their classroom after lunch and prepared for their scheduled rest/nap time. Several children were dismissed early from the program in advance of the majority. Prescribed medications were observed for 2 children, written parental/physician authorizations to administer were current. A sample of 3 children's records was reviewed and one partial staff record. Documentation of the Center's practiced emergency drills was current; the required emergency supplies were on hand. Complete staff records are maintained off site, this LI reviewed two staff records at the administration office on March 10, 2017. All information in the records was complete and current. Standard #: 22VAC40-185-60-A Description: Based on a sample review of 3 children's records, the Center failed to maintain complete information in each record. The following required information was not on file: 5.) name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached; 2.) the name, home address, and home phone number of each parent who has custody; 3.) when applicable, work phone number and place of employment of each parent who has custody. Evidence: Information was incomplete in two of the three reviewed records. 1. The records of children 1 and 3 did not have at least 2 identified emergency contacts on record. Child 3's parent is listed as one of the two emergency contacts. 2. The name, home address and home phone number and when applicable, work phone number and place of employment of each custodial parent/legal guardian was not listed in the record of child 1. Plan of Correction: The responsible staff member will review the records with each child's parent and will request that complete information be provided for record retention.
