Technical Assistance:
Technical assistance was provided in the following areas: 22VAC40-185-(2)-70-A (Staff records); 22VAC40-185-(4)-330 (Play areas)

Comments:
An unannounced monitoring inspection was conducted on 3/31/17 from 9:25am - 11:45am. During the inspection there were 42 children ages two years to five years old in care with 12 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms including arts and crafts, circle time, and eating morning snack. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member has documentation of a negative tuberculosis screening within 21 days after beginning employment and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. The record for staff #1 (date of hire 1/3/17) contained documentation of a negative tuberculosis screening that was dated 1/12/15. 3. Staff #4 (Program Director) confirmed the TB screening for staff #1 was not completed within 12 months of employment.

Plan of Correction: The facility responded: Staff #1 will get a TB screening. All new staff will complete a TB screening within 21 days of employment, unless they have a TB screening that was completed within the last 12 months.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of five staff records, it was determined that the facility did not ensure that staff provide before three years since the dates of the most recent central registry finding and most recent criminal history record check report Evidence: 1. The record for staff #3 contained a completed criminal history record check report dated 1/15/14. 2. The record for staff #4 contained a completed central registry finding dated 12/18/13.

Plan of Correction: The facility responded: The updated background checks for staff #3 and staff #4 have already been mailed and we are waiting to receive them. The Licensing Inspector was shown information to demonstrate that the new back grounds checks had been completed by the staff and were mailed off to be processed.
