Today we discussed emergency prepardness plans
An unannounced renewal inspection was conducted today from 9am through 12:45pm. There were 24 children present with five staff. Children and staff records were reviewed. Required postings, fire and health inspections, injury report, evacuation drills, shelter-in-place drills and insurance documentation was reviewed. Children were observed in teacher and self-directed activities including lessons, restroom routines, hand washing, outside play, lunch and nap. Violations regarding tuberculosis screening/test and annual training hours were observed and appear on the violation notice.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview the center failed to ensure that at least every two years from the date of the first initial screening staff members obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1 - The records for staff 1 - 4 contain documentation of tuberculosis screenings or test that are past due for update. a. The date of the last documented tuberculosis screenings or test for staff 1 is dated 8/7/12. b. The date of the last documented tuberculosis screenings or test for staff 2 is dated 9/7/14. c. The date of the last documented tuberculosis screenings or test for staff 3 is dated 9/7/14. d. The date of the last documented tuberculosis screenings or test for staff 4 is dated 9/7/14. 2 - Staff 1 and 2 confirmed that the tuberculosis screenings or test in the staff records are past due for update. Plan of Correction: Going forward center management will create and implement a system for alerting staff prior to expiration that the screening/test needs to be updated. The center notes that management staff was confused documentation provided by the doctor's office which included an expiration date. However, that date was regarding the test serum and not the licensing requirement for updates every two years.
Standard #: 22VAC40-185-240-C Description: Based on record review and interview the center failed to ensure that program directors and staff who work directly with children annually attend 16 hours of staff development activities that shall be related to child safety and development and the function of the center. Evidence: 1 - Four out of five staff records reviewed contain documentation for 8 hours of staff development training obtained during the 2016 calendar year. 2 - Staff 2 stated that training hours calculated January through December each year. 3 - Staff 2 confirmed that each staff record contained only 8 hours of training where 16 annual hours are required. Plan of Correction: Going forward center management will ensure that each staff member obtains the required number of annual training hours.
