Comments:
An unannounced monitoring inspection was conducted today from 7:10am-8:00am. There were 13 school age children directly supervised by 3 staff. The physical plant, 4 staff training records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed studying space, creating lego designs, playing cards, doing trivia games, and reading. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the facility failed to maintain children's records with the required information. Evidence: Child #1 did not have two emergency contacts completed. Their second emergency contact was a Post Office box.

Plan of Correction: Corrected at the time of inspection. Staff called the parent during the time of inspection and the parent said the address was not a Post Office box, but she will write the address more clearly. (SIC)
