The licensing inspector conducted an unannounced monitoring inspection Friday, April 7, 2017 from approximately 10:15am to 3:00pm. A member of management was present to assist with the inspection. The following ratios were observed with a total of 74 children present during the inspection:
Infants: 5 children with 2 staff
Toddlers: 8 children with 2 staff
Twos: 13 children with 2 staff
Preschool I: 6 children with 1 staff
Preschool II: 20 children with 2 staff
Pre-K: 22 children with 3 staff
School age children were not present during the inspection.
During the inspection, children and staff were observed participating in a variety of activities. The Infants were observed playing on the floor with toys, sleeping in their cribs, being held and rocked. The Toddlers were observed taking a stroll in a buggy and eating lunch. The Twos were observed playing outside, looking at books, taking bathroom breaks and washing hands to prepare for lunch. The Preschool I class was observed playing outside, playing with play-doh, building with blocks and playing in the dramatic play area. The Preschool II classroom was observed playing outside, using lacing blocks, playing with plastic animals, building with legos, looking at books and playing in the dramatic play area. The Pre-K children were observed participating in circle time where a review of colors, letter, number and pointing and naming different body parts took place. Later the children played outside. Children were observed eating lunch which consisted of ham and cheese roll-ups, salad, bananas and milk. Later children transitioned into nap time. All areas of the facility including classrooms, hallways, outdoor play area and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children.
The following information was reviewed: injury reports, daily attendance, daily sheet, infant feeding schedules, emergency preparedness plan, practiced emergency drill logs, written allergy plans, menu, and staff training information. Required postings were on display in the center. Medication is administered when required and medication and medication authorizations were reviewed. Transportation is provided and the bus and transportation items were inspected.

Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, two records did not contain the required information.
During the inspection, the licensing inspector observed the updated information in the record for Child #3 (enrollment date 06/02/14) listed one of the two required emergency contacts. The record for Child #4 (enrollment date 08/29/16) contained two listed emergency contacts and their phone number but did not list the physical addresses.
Plan of Correction: We will reach out to the families and obtain the required information and add it to the record.
Standard #: 22VAC40-185-540-C Description: Based on observation, the facility did not ensure all required items were located in the first aid kit.
During the inspection, the licensing inspector observed the first aid kit located on the bus did not contain tweezers and a thermometer. The bus is used to transport children to and from public school.
