Comments:
An unannounced monitoring inspection was conducted on April 13, 2017. There were 117 children present at arrival to the facility in eight classrooms with 16 staff members and three volunteers. Additional staff members and volunteers were on site for administrative and support purposes. The children were observed having lunch which consisted of cube steak, mashed potatoes, mixed vegetables, bread, apricots and milk. The children were also observed napping and during circle time in one classroom in which they participated in a Letter M recognition activity and storytime. The outdoor playground equipment is not being utilized at this time. Five children's records were reviewed. Eight medications were reviewed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in four of five children's records reviewed, the licensee did not maintain and keep at the center a separate record for each child enrolled which contained all required information. Evidence: During the inspection conducted on April 13, 2017, the records for Child #1, Child #2, Child #4 and Child #5 did not include documentation of viewing proof of the child's identity and age.

Plan of Correction: I will speak to the Family Services Supervisor.

Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on April 13, 2017, the fire inspection report for the facility had expired and was dated 9/14/15.

Plan of Correction: A fire inspection will be scheduled.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on April 13, 2017, there was a bottle of correction fluid labeled keep out of reach of children of children and caution/extremely flammable on a shelf in Room 103.

Plan of Correction: Talked to staff and reminded of how to properly store hazardous materials.

Standard #: 22VAC40-185-540-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that each first aid kit shall be easily accessible to staff but not to children. Evidence: During the inspection conducted on April 13, 2017, the licensing inspector observed two first aid kits on a shelf in Room 104. There was a chair positioned directly in front of the shelf making the kits accessible to the children in care.

Plan of Correction: Will speak to staff about properly storing first aid kits.
