An unannounced inspection was initiated in the facility on April 5, 2017, regarding staffing and supervision and special care provisions and emergencies. The inspection included observations, a review of center documentation and interviews with staff members . Based on information obtained during the inspection the preponderance of evidence does support the allegation related to staffing and supervision therefore the complaint is determined to be VALID. Areas of non compliance are noted on the violation notice.

Description: Based on staff interviews and attendance documentation, the licensee did not ensure that for infants, there was one staff member for every four children. Evidence: During a complaint inspection on April 5, 2017, the licensing inspector reviewed head count forms for the infant classroom from ages 0-16 months. The center did not comply with the 1:4 ratio. - On February 23, 2017 in the infant classroom, there was 1 staff with 5 children at 7:17am to 7:31am from ages 3 months to 11 months . - On March 14, 2017 in the infant classroom, there was 1 staff with 6 children at 6:47 am to 7:08 am from ages 4 months to 9 months . - On March 16, 2017 in the infant classroom, there was 1 staff with 5 children at 7:03 am. At 7:07am -7:14am there was 1 staff with 6 children from ages 7 months -10 months. - On March 17, 2017 in the infant classroom, there was 1 staff with 5 children at 6:25am- 6:43am from ages 6moths -16 moths. At 6:43am - 6:45am there was 1 staff with 6 children from ages 4 months -18 months. At 6:45am to 7:05am there was 1staff with 5 children from ages 4 months-9months.

Plan of Correction: Management is being retrained on or before April 18, 2017 regarding proper steps to follow when there are staff call outs and she is unable to staff her building appropriately. DM will oversee weekly hiring updates to ensure that the school has proper staffing levels at all times.
