Technical Assistance:
Technical assistance was provided in the following areas: 22VAC40-185-(4)-330 (Play Areas); 22VAC40-185-(6)-550 (Emergency Procedures)

Comments:
An unannounced monitoring inspection was conducted on 4/18/17 from 8:55am - 11:15am. During the inspection there were 30 children ages three months old to four years old in care with 15 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, eating lunch, and during rest period. Records were reviewed for four children and seven staff/board members. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of four children's records and interviews, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #4 (date of enrollment 3/24/17) did not contain documentation of viewing proof of the child's identity and age. 2. Staff #1 (Program Director) confirmed that there was no documentation available for viewing during the inspection to demonstrate that the facility had viewed proof of the child's identity and age.

Plan of Correction: The facility responded: The parent of child #3 was called during the inspection and will bring the proof of birth letter today.

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the facility did not ensure that a nonabsorbent surface for diapering or changing is used. Evidence: 1. The changing pad on the diaper changing table in the Infant classroom had a tear on the top of the pad. 2. Staff #1 (Program Director) confirmed that there was a tear in the diaper changing pad.

Plan of Correction: The facility responded: A new diaper changing pad will be purchased and placed on the Infant room changing table.
