Technical Assistance:
During inspection, inspector informed staff on site about the medication authorization which had an expiration date of "none"; Staff must touch based with parents annually to make sure that allergy and medical needs of children are up to date and accurate.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:00am-1:30pm. There were 37 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating lunch. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Description: Based on a review of a sample of children's records all required information was not maintained. Evidence: During inspection Child #4's record was missing 1 parent's work phone number.

Standard #: 22VAC40-185-270-A
Description: Baed on inspection of Child Day Center all equipment was not in operable condition. Evidence: During inspection the trash in the middle bathroom on the main floor did not have a working foot pedal.

Plan of Correction: The Trash can was discarded and replaced

Standard #: 22VAC40-185-280-B
Description: Based on inspection of Child Day Center hazardous substances were not kept in a locked place. Evidence: During inspection, 1. the Laundry room in the infant area was opened and unlocked with detergent on the floor. 2. In the bathroom of the infant area air freshener and cleaning supplies were stored on a shelf and on the sink 3. In the main level bathroom cleaning wipes and air freshener were out on the back of the toilet and sink.

Plan of Correction: We will remind the entire staff to maintain the door properly locked at all times.
