Technical Assistance:
Discussed Subsidy requirements and record keeping for Subsidy.

Comments:
An unannounced inspection was conducted from 1025- 1130 today. There were 19 children with 8 staff actively supervising the children. There were also 2 staff present handling administrative responsibilities. The Director was on vacation but the Health Coordinator was present, available, and cooperative. The Inspector was able to review records, observe the program and check special care provisions. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and observation of allergy documents posted, the center is not ensuring that when food allergies are listed on boards and recorded in files, that a plan of action is included in the event the child comes into contact with allergen. Evidence: 1. Three children's names were posted on most current Allergy list for staff boards. 2. The Inspector noted the allergies listed and requested the children's documents. 3. All three files did not include a plan of action in the event the child came in contact with the allergens.

Plan of Correction: I will request the most up-to-date food allergen listed from Nutritionist and also follow up with health coordinator to obtain the FAIR form from the child's Pediatrician.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas inside the center were maintained. The bathroom walls showed signs of chipping paint along the corner.

Plan of Correction: We will contact the facilities Director to request paint issue be corrected.

Standard #: 22VAC40-185-570-E
Description: Based on observation the center has not ensured that all infant's bottles are labeled and dated. Evidence:: 1. The Inspector observed a small infant's bottle full and sitting on the counter. There was no name on the bottle and further review, found it did not contain a date.

Plan of Correction: Nutritionist will retrain the staff.
