VENDSUB-000 SPECIAL CARE PROVISIONS AND EMERGENCIES

Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on April 20, 2017 from approximately 4:25 p.m. until 6:30 p.m. Upon entrance there were 43 children and 3 staff present with children. Children were observed eating snack, and playing in centers to include basketball, Legos, board games. The record review sample consisted of 4 children's records and 3 staff records. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, within 5 days of receipt. Violations:
Standard #: VENDSUB-000-012-B
Description: Based on observation, the vendor did not ensure within the first 90 days of employment or subsidy vendor approval that all staff have completed the Virginia Preservice Training for Child Care Staff. Evidence: During the inspection on April 20, 2017, the records for three staff were reviewed and it was determined there was not documentation the staff #1 and #2 had completed the Preservice training. The center supervisor verified this during the inspection.

Plan of Correction: The Center Supervisor will ensure that all staff files are up to date to include include training and certification documentation.

Standard #: VENDSUB-000-012-F
Description: Based on observation, the vendor did not ensure staff employed by an approved vendor prior to the effective date of the vendor agreement completed CPR and first aid training as required by this section within 90 days of the effective date of this agreement. During this 90 day period, there must always be at least one staff with current CPR and first aid training present during operating hours of the center. Evidence: During the inspection on April 20, 2017, the records for three staff were reviewed and it was only one of the staff had documentation indicating they had obtained the CPR and first aid training . The center supervisor verified this during the inspection.

Plan of Correction: Newport News PR&T maintains CPR & first aid certifications of all full time staff. Administration will continue to work toward certification of all other staff.

Standard #: VENDSUB-000-031-B-1
Description: Based on observation, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during an evacuation. Evidence: During the inspection on April 20, 2017, there were not reunification plans documented . The center supervisor verified this during the inspection.

Plan of Correction: The Center Supervisor will ensure that reunification plans are included in the emergency evacuation plans,
