The licensing inspector conducted an unannounced monitoring inspection Monday, April 24, 2017 from approximately 10:15am to 1:10pm. The following ratios were observed with a total of 144 children present during the inspection:
Satellite Infants - 4 children with 1 staff
Young Infants - 8 children with 2 staff
Older Infants -7 children with 2 staff
Toddlers (Red Room) - 15 children with 3 staff
Twos (Orange Room) - 16 children with 3 staff
Twos & Threes (Yellow Room) - 19 children with 3 staff
Threes (Green Room) - 19 children with 2 staff
Threes & Fours (Blue Room) - 17 children with 2 staff
Fours & Fives (Purple Room) - 20 children with 2 staff
Fours & Fives (Gold Room) - 19 children with 2 staff
Get Well ? no children were present during the inspection
During the inspection, children and staff were observed participating in a variety of activities. The Infants were observed playing on the floor with toys, being held and fed and eating lunch. The Toddlers were observed singing songs, having their diapers changed, washing hands and eating lunch. The Twos were observed eating lunch. The Threes were observed playing outside, building with blocks, singing sounds and transitioning into lunch. The Pre-K children were observed leading activities based on INSPIRE lesson plans, playing outside, participating at circle time and eating lunch. Lunch consisted of turkey sandwiches, corn, apples and milk. Later children transitioned into nap time. All areas of the facility including classrooms, hallways, outdoor play area and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children.
The following information was reviewed: injury reports, daily attendance, daily sheet, infant feeding schedules, practiced emergency drill logs, written allergy plans, menu, liability insurance and staff training information. Required postings were on display in the center. Medication is administered when required and medications and medication authorizations were reviewed.

Standard #: 22VAC40-185-280-B Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed a container of disinfecting wipes and a container of glass cleaner in the cabinet located in the children's bathroom. The cabinet contained a child's lock on it but due to the clothes bin inside of the cabinet, the cabinet door could not shut, preventing the door from locking. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-191-40-D-1-C Description: Based on review of five staff records, one record did not contain a current sworn statement. Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 07/30/2010) contained a sworn statement dated 07/11/2013. Sworn statements are required to be updated at least every three years from the date on the previous sworn statement. Plan of Correction: Per director - I will have the staff complete an updated sworn statement and add it to the record.
