Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed a container of disinfecting wipes and a container of glass cleaner in the cabinet located in the children's bathroom. The cabinet contained a child's lock on it but due to the clothes bin inside of the cabinet, the cabinet door could not shut, preventing the door from locking.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-191-40-D-1-C

Evidence: During the inspection, the licensing inspector observed the record for Staff #2 (hire date 07/30/2010) contained a sworn statement dated 07/11/2013. Sworn statements are required to be updated at least every three years from the date on the previous sworn statement.

Plan of Correction: Per director - I will have the staff complete an updated sworn statement and add it to the record.
