Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, April 25, 2017 from approximately 3:45pm to 5:40pm. There were a total of 18 children being supervised by three staff members during the inspection. The children and staff were observed participating in a variety of activities. Children were observed eating snack, completing homework, looking at books, playing video games, playing restaurant from ?The Green Turtle? restaurant the children designed, arranging floral bouquets and building with legos. Staff was observed playing games with the children, sitting with the children and eating snack and helping children with homework. All areas of the facility were equipped with equipment and toys for the children to use.

The following information was reviewed: daily attendance, emergency preparedness plan, menu, emergency drills, emergency supplies, liability insurance policy, transportation documents, and parent agreements. The required information was observed posted in the facility. Management stated medications were not present at the facility.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of three staff records and interview, the center did not ensure the required staff information was present at the facility. Evidence: During the inspection, the licensing inspector asked a member of management if each staff member had documentation of an emergency contact information and information regarding any possible medical conditions staff may have present at the facility. Staff stated this information was not present at the facility. Later during the inspection, staff files were brought to the facility and the information was in the records.

Plan of Correction: Copies of the required information was made while the licensing inspector was on site to be kept at the facility.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure all areas were maintained in a safe and operable condition. Evidence: During the inspection, the licensing inspector observed the left side of wall along the children's bathroom door frame contained peeling paint.

Plan of Correction: The owner took a picture of the peeling paint and sent it to the landlord. Work to fix the area will be completed.

Standard #: 22VAC40-185-550-E
Description: Based on documentation review, the facility did not ensure all practiced drills were documented. Evidence: During the inspection, the licensing inspector viewed the emergency drill logs. The last documented evacuation drill was documented on 11/11/16. A member of management stated the monthly drills have been completed but the information needs to be documented.

Plan of Correction: We will document the drills as they are completed.
