Comments:
An unannounced monitoring inspection was conducted on April 20, 2017. At the time of entrance there were 47 children and 4 staff members present with children. A sample size consisted of 4 children's records and the emergency information and health related information related to the job position for 4 staff members. Children were observed eating snack and playing games. The inspection began at approximately 4:25 p.m. and ended at 6:30 p.m.. Areas of non-compliance were noted on the violation notice. The results of the inspection were reviewed and verified with the center supervisor. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return it to the licensing office within 5 days of receipt. Standard #: 22VAC40-185-130-A
Description: Based on observation and record review the licensee did not ensure there was documentation indicating each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: During the inspection on April 20, 2017, the record for child #4 did not include a copy of immunizations. The child's first day of attendance was 9-6-2016 as confirmed by the center director.

Description: Based on observation and record review the licensee did not ensure there was documentation indicating all children had a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: During the inspection on April 20, 2017, the record for child #4 did not include a copy of physical. The child's first day of attendance was 9-6-2016 as confirmed by the center director.
