Technical Assistance was provided in the following areas of the standards: 22VAC40-185- (4)-270-A (Building Maintenance); 22VAC40-185-(7)-550 (Emergency Drills)
An unannounced monitoring inspection was conducted on 4/28/17 from 9:10am - 12:45pm. During the inspection there were 39 children ages three months old to four years old in care with eight staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, eating lunch, and during rest period. Records were reviewed for five children and six staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.
Standard #: 22VAC40-185-160-C Description: Based on a review of six staff records, it was determined that the facility did not ensure that all staff shall obtain and submit the results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening or testing. Evidence: 1. The record for staff #3 contained a TB screening dated 8/1/14. 2. The record for staff #4 contained a TB screening dated 4/6/15. 3. Staff #5 (Program Director) that an updated TB screening had not been received for either staff. Plan of Correction: The facility responded: The staff will each get an updated TB screening and it will be placed in their file.
Standard #: 22VAC40-185-70-A Description: Based on a review of six staff records, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence: 1. The record for staff #6 (date of hire 8/24/16) did not contain documentation of that two or more references were checked before employment. 2. Staff #5 (Program Director) reviewed the record for staff #6, and confirmed there was no documentation for references available for viewing during the inspection. Plan of Correction: The facility responded: New references will be completed for staff #6 and it placed in the file.
