22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today. Observed 46 children directly supervised by 9 staff members. Ratios and supervision were in compliance. A sample of 3 children's records and 3 staff records were reviewed. The physical plant, medications, emergencies supplies and procedures were also reviewed. The children were engaged in various activities, to include: free play, reading, circle time discussing gardening and what types of foods the children grow at home, infant care, feeding and diapering. Areas of non compliance are identified in this report. The exit interview was conducted with the Director. Inspection conducted 9:45 am-12:30 pm. Standard #: 22VAC40-185-160-C Description: Based upon record review, staff did not resubmit TB test results every 2 years as required. Evidence: Staff A did not have a resubmitted TB test result on file. Last Tb on file 01/16/2015-01/2017. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-60-A Description: Based upon records reviewed, required information for children's records was found to be incomplete or missing. Evidence: Child A's record is missing 2 complete addresses for emergency contact #1 and #2. Child B's record is missing updated immunization records to be provided at the time of enrollment and every 6 months for a child under the age of 2. Child B's start date 7/20/2015. Child B's record is also missing complete addresses for emergency contact #1 and #2. Plan of Correction: Center director will ask parent to provide updated immunization records at pick up.
Standard #: 22VAC40-185-280-B Description: Based upon observation and interview, hazardous substances were not kept in a locked place. Evidence: The cabinet where cleaning products are stored in preschool classroom did not have a working locking mechanism to prevent access by children in care. Plan of Correction: A work order will be submitted to fix locks.
