Comments:
An unannounced monitoring insepction was conducted from 10:30 am - 12:00pm. There were 38 children in care being supervised by five staff members. The children were observed participating in structured center activities. Lunch is provided from home. Ten children's records were reviewed and in compliance. Three staff records were reviewed and in compliance. Three accident reports were reviewed and in compliance. One medication was reviewed and in compliance. There are currently two staff members on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 4/28/2017 and the most recent shelter in place drill was conducted 4/6/2017. Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground structure it was determined that not all fall zones maintained the proper resilient surfacing. Evidence: The resilient surfacing (wood chips) had dispersed from the area at the bottom of both slides and did not meet the minimum 6 inches required.

Plan of Correction: Administration will ensure that all fall zones have a minimum of six inches of resilient surfacing.
