Comments:
An unannounced health and safety inspection was conducted from 10:05 a.m. - 1:10 p.m. There were forty-five children present with nine teachers working directly with them. Four children's records were reviewed. Two teachers records were reviewed. The younger children were observed during tummy time, playing with toys, being fed, talked to and played with by the teachers and sleeping. The older children were observed during snack, hand washing, singing and dancing, playing at various stations the teachers had prepared ahead of time for them, stuffing pillows for Mother's Day and laughing and talking with one another. Description: Based on record review and documentation the facility failed to have written allergy plans for four children. Evidence: 1. On 05/11/2017 the licensing inspector and staff #1 reviewed all information regarding food allergies for four children. Although there was information from the physicians there was no written allergy care plan for child #1, child #2, child #3 and child #4. 2. On 05/11/2017 staff #1 confirmed there were no written allergy plans for child #1, child #2, child #3 and child #4.

Plan of Correction: We will be reviewing two different care plans and may make some revisions today to the one we choose to use. We then will request each parent to have these care plans completed by the physician. We will post the care plans in the room with the child's picture if the parent permits.
