Comments:
An unannounced monitoring inspection was conducted from 10:05 a.m. - 4:45 p.m. There were twenty-one children in care with two teachers. Three children's records were reviewed. The record for one teacher was reviewed. The children were observed coming in from school for care, playing board games with one another, individually reading, building with legos, working on homework, during hand washing, snack, clean-up and during quiet time. Violations:
Standard #: 22VAC40-185-260-B
Description: Based on a review of documentation the facility failed to have documentation of an annual approval from the health department. Evidence: 1. On 05/11/2017 the licensing inspector reviewed the most current health inspection which was 02/23/2016 with staff #1. 2. On 05/11/2017 staff #1 confirmed this was the most recent health inspection.

Plan of Correction: I will telephone the health department immediately and request the health inspection. I will send a copy of our inspection to the licensing inspector for the record.

Standard #: 22VAC40-185-550-E
Description: Based on interview the facility failed to maintain a record of the dates of practice drills for one year. Evidence: 1. On 05/11/2017 the licensing inspector requested to see the monthly evacuation drills and shelter-in-place drills. Staff #1 advised she could not find the documentation.

Plan of Correction: We recently rearranged the club house in order to provide more space for the children. I suspect I will find this document at a later time. I will from today forward keep the records as per the standards.
