Comments:
An unannounced Annual Subsidy Health and Safety Inspection was conducted from 11:00 a.m. to 5:15 p.m. The Vendor is currently in the 90 day initial period as a new subsidy vendor. At the time of entrance the following adult to child ratios were observed: 13 children, ages 2 to 4 years old with 1 staff 5 infants, 8 months to 1 year old, with 2 staff The records sample size consisted of 3 children?s records and 4 staff records. Children were observed playing with toys, painting, eating lunch, singing songs, resting quietly and having an afternoon snack. Infants were observed having tummy time, sitting in infant chairs, toddling and walking, playing with toys, drinking bottles and sleeping. Restroom procedures, hand washng, diapering procedures, and table sanitizing were observed. Medication administration was not observed. The licensee states that no medication is administered and children are not routinely transported in a vehicle. There are currently no children enrolled who have allergies, food sensitivities, or dietary restrictions. Areas of non-compliance are identified on the violation notice. Standard #: VENDSUB-000-005-B
Description: Based on observation and interview, the licensee did not maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: The sign in and out sheet used by parents to document the arrival and departure times of their children showed 11 children in care at 11:45 a.m. There were 20 children observed in care at that time. Staff D. confirmed that this is the only written documentation of the arrival and departure times of the children in care.

Plan of Correction: Staff D. stated that an attendance log will be kept up by staff in each classroom, which documents the arrival and departure of each child in care as it occurs, beginning Monday.
