Technical Assistance:
Inspector spoke with the Director about 22VAC40-185-540 and 22VAC40-185-510 during this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 10:30 am-11:45 am. At the time of inspectors' arrival there were 77 toddler-6 year old children in care with 8 staff members. Children were observed participating in Montessori activities/instructional time in the classroom, transitioning between activities and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are noted on the violation notice and were discussed with the Director during exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: In the foyer to Children's House I there was a large shelving unit used for shoes. This shelving unit was unstable and presented a potential tipping hazard.

Plan of Correction: All shelving units will be anchored.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined that the center did not ensure that all hazardous substances were kept in a locked place. Evidence: In the Toddler classroom there was dish washing soap and disinfectant wipes that had "Keep out of the reach of children" labels. in an unlocked basket over the sink.

Plan of Correction: All chemicals will be locked.

Standard #: 22VAC40-185-290-3
Description: Based on observation it was determined that the center did not ensure that all unused electrical outlets had protective covers. Evidence: There was an open outlet on an extension cord in the Toddler room.

Plan of Correction: Outlet will be capped.

Standard #: 22VAC40-185-560-G
Description: Based on observation it was determined that the center did not ensure when food is brought from home the container is clearly dated. Evidence: 3 out of 14 lunch boxes viewed were not dated.

Plan of Correction: Staff will ensure all lunch boxes are dated.
