Comments:
An unannounced inspection was conducted on May 11, 2017 at approximately 10:45 am - 1:15pm . At the time of entrance there were seventy -four children and fourteen staff members present with children. Additional administrative staff was present to assist. The sample size consisted of 4 children records, 9 staff records, 3 injury reports were reviewed and 2 medication. Children were observed learning activity on tablets , making mother's day activity, station activities and story time. Areas of non-compliance are identified on the violation notice. Exit interview conducted with program director.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review and staff interviews, the licensee did not ensure there was a separate record for each child enrolled which contained all the required information. Evidence: During the inspection on May 11, 2017, the records for the following children were determined to be incomplete. The record for child #3 did not include employer for one parent and the employment phone numbers for both parents listed .

Plan of Correction: Missing information was obtained the day of the visit. Management will ensure that no child starts without completed info as required.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on May 11, 2017, the licensing inspector observed the following: -In the 2 year old classroom there was red peeling paint on the inside of the bathroom door. -In the Pre-K classroom there were multiple areas with peeling paint. - In the Pre-K bathroom, there was a protruding screw in the bathroom on the back wall next to the the toilet.

Plan of Correction: We are working in conjunction with our facilities team to have these areas repainted on or before 6/30/17. Moving forward our Safety Captain will be monitoring for chipped paint during weekly safety checklist monitoring.

Standard #: 22VAC40-185-550-M
Description: Based on documentation review, the licensee did not ensure each there was a written record of children's serious and minor injuries which entries were made the day of occurrence and included all the required information. Evidence: During the inspection on May 11, 2017, 2 of 3 injury reports were reviewed did not include the following: -May 2, 2017 injury record did not have staff and parent signatures or two staff signatures - May 3, 2017 injury record did not have time parent was notified and how the the parent was notified of the injury.

Plan of Correction: All staff and management will review incident/accident reporting requirements on or before 5/30/17. DM will review these forms quarterly for compliance.
