Technical Assistance:
Today we reviewed standards regarding USDA meal portions and food groups and confidentiality of records.

Comments:
An unannounced Monitoring Inspection was conducted today from 11:45am through 2:45pm. There were 38 children in care with five staff. Children present ranged in age from 12 months to four years old. Children were observed in teacher and self-directed activities including circle time, table top toys/play, restroom routines, hand washing, lunch and nap. Children and staff records were reviewed. Required postings, emergency evacuation drills, emergency supplies and required inspections were reviewed. Violations observed during the inspection include staff records, tuberculosis screening/test, and central registry finding. These violations appear on the violation notice.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and interview the center failed to ensure that at least every two years from the date of the first initial screening or testing, staff obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section. Evidence: 1 - During record review the record for staff 8 does not contain documentation of a current tuberculosis screening or test. a. The date of the tuberculosis test on file for staff 8 is 3/6/15. There the test is past due for update. 2 - Staff 1 confirmed that the tuberculosis screening on file for staff 8 is pasted due for update.

Plan of Correction: Center management will ensure that all staff have updated tuberculosis screening or test every two years as required.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information. Evidence: 1 - The record for staff 8 does not contain documentation of written information to demonstrate that the individual possesses the education, training, staff development, and certification, required by the job position. a. Staff 8 was present working in the toddler care room during the inspection today. b. There is not documentation that demonstrates that staff 8 is qualified as a program leader 3 - The record for staff 8 contains job titles and descriptions for teacher assistant and cook but not program leader. 2 - Staff 1 stated that staff 8 was the program leader in the toddler room.

Plan of Correction: Center management will ensure that each staff record clearly documents how each staff is qualified for the position they are working.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview the center does not have a central registry finding within 30 days of employment. Evidence: 1 - During review of staff records the record for staff 7 does not contain documentation that the central registry finding was requested or received within the allotted time frames. a. The documented date of hire for staff 7 is 4/22/17. 2 - Staff 1 confirmed that the record for staff 7 does not contain documentation that the central registry finding was requested.

Plan of Correction: Center management will ensure documentation of background checks is clearly written in each staff record.
