Comments:
An unannounced Subsidy Health and Safety Inspection was conducted today from 11:45am through 2:15pm. There were 38 children in care with five staff. Children present ranged in age from 12 months to four years old. Children were observed in teacher and self-directed activities including circle time, table top toys/play, restroom routines, hand washing, lunch and nap.

Violations:
Standard #: VENDSUB-000-012-C
Description: Based on record review and interview the vendor failed to ensure that staff employed prior to the effective date of this agreement have completed the Virginia Preservice Training for Child Care Staff, to include all of the elements applicable to new staff, within 90 days of effective date of this agreement. Evidence: 1 - During review of nine staff records, the records for staff 1,2, 3, 5 and 6 do not have documentation of completing the required Preservice Training. 2 - Staff 1 confirmed that the staff do not have documentation of completing the required training on file.

Plan of Correction: All staff will have documentation of completing the required preservice training by the end of the this month.

Standard #: VENDSUB-000-012-F
Description: Based on record review and interview the vendor failed to ensure that staff have current CPR and first aid. Evidence: 1 - During record review the records for staff 2, 4, 5 and 6 do not have documentation of current first aid and CPR as required. a. Staff 2 has a documented date of hire as 9/8/15. b. Staff 4 has a documented date of hire as 2/17/16. c. Staff 5 has a documented date of hire as 12/28/16. d. Staff 6 has a documented date of hire as 9/9/15. 2 - Additionally, the center vendor agreement was signed on 12/2/16 more than 90 prior to today's inspection. 3 - Staff 1 confirmed that all staff have not completed CPR and first aid training.

Plan of Correction: All staff will have current CPR and first aid training by the end of June 2017.

Standard #: VENDSUB-000-031-B-1
Description: Based on procedure review and interview the vendor failed to ensure that evacuation procedures address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: 1 - During review of emergency procedures the vendor does not have a written plan to ensure reunification of children with parents during an emergency evacuation of the facility. 2 - Staff 1 confirmed that there is not a written reunification plan available for review.

Plan of Correction: Center management will write, implement and train staff in the facilities procedure to reunite children with parents after an emergency.
