An unannounced renewal inspection was conducted on Tuesday, May 23, 2017, from approximately 2:30pm until 5:45pm. The following classroom staff to child ratios were: Infants - four infants with one staff Mobile Infants - seven children with two staff Toddlers - nine children with two staff Two's - fifteen children with two staff Three's - fourteen children with two staff Four's - thirteen children with two staff School age - seventeen children with two staff Observations included nap time, afternoon snack, diapering and activities. The children have completed "hand print" crabs, and created "brick" paint art. Children were building with various maniplatives, lacing beads, listening to stories, washing plastic animals and having frequent conversations with staff. The last practice evacuation drill was conducted 4/4/17. A practice "lock down" drill was conducted 5/18/17. PM Snack: Animal crackers and milk All staff are currently certified in first aid and CPR. Seventeen of the nineteen staff employed are currently certified in Daily Health Observation. Five children's records, four staff records and three medications were inspected.
Standard #: 22VAC40-185-160-A Description: Based on inspection of staff records, the center did not ensure documentation of a negative tuberculosis (TB) were dated no more than 12 months prior to or 21 days after employment.
Staff #1 started working 4/10/17 and submitted a TB documented 10/19/15.
Staff # 4 started working 3/13/17 and submitted a TB documented 7/28/15.
Plan of Correction: Per Center: Staff #1 and Staff #4 will be required to obtain an updated TB.
Standard #: 22VAC40-185-270-A Description: Based on inspection of the facility, the center did not ensure all areas and equipment of the center were maintained in a clean,safe, and operable condition. Evidence: The red, plastic couch cushion is torn in the three year old classroom. The carpet in the school age classroom and a carpet in the three year old classroom is not secure and poses a tripping hazard. Plan of Correction: Per Center: The carpets were immediately removed. The red cushion will be removed.
