An unannounced monitoring inspection was conducted on 5/23/17 from 2:25pm - 4:45pm. At the time of entrance there were 42 school age children in care with five staff. An additional 54 children arrived from local public schools along with four additional staff. Children were observed eating snack, completing homework, and participating in various activities in the cafeteria, gym and classrooms. Records were reviewed for five children while at the facility. The records for five staff were reviewed on 5/24/17 at the central office. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.
Standard #: 22VAC40-185-140-A Description: Based on a review of five children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child#1 (date of enrollment 9/15/16) did not contain documentation of a physical examination. 2. The record for child#5 (date of enrollment 9/14/16) did not contain documentation of a physical examination. 3. Staff #1 (Program Director) stated that they had received physicals for each child, but that he could not provide documentation to confirm that during the inspection. Plan of Correction: The facility responded: The parents of both children were asked to provide a new copy of their children's physicals.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication being stored at the facility, it was determined that the facility did not ensure that they follow their procedures for administering medication. Evidence: 1. The Licensing Inspector a plastic bag with medication (Ventolin inhaler) for Child #5 in the medication storage box. 2. There was no written authorization from the parent available for viewing during the inspection. 3. Staff #1 (Program Director) stated that they had received written authorization from the parent to administer medication to child #5, but he could not provide the document during the inspection. Plan of Correction: The facility responded: The parent of child #5 was called during the inspection and asked to complete a new medication authorization.
