Comments:
An unannounced health and safety inspection was conducted from 9:20 a.m. - 1:00 p.m. There were fifty-four children in care with eleven teachers. Two other facility staff were available to assist as needed. Four children's records were reviewed. Three teachers records were reviewed. The children were observed playing, talking and laughing, watching and listening to a Smurf presentation during quiet time and sleeping. Description: Based on a review of documentation the facility failed to have a written allergy care plan for one child who is enrolled. Evidence: 1. On 05/26/2017 the licensing inspector and staff #1 reviewed the allergy information for child #1 and agreed there was no written allergy care plan for the child.

Plan of Correction: We have been using the USDA Food Program Dietary Prescription Form dated and signed by the physician for the child. We will request that the parent obtain a written allergy care plan. We can notify the parent immediately and will request this information be provided for the record by Tuesday.
