22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:20 a.m. - 3:45 p.m. There were fifty-four children in care with eleven teachers working directly with them. There were two other staff available to assist as needed. Four children's records were reviewed. The record for three teachers were reviewed. The younger children were observed being rocked and comforted by the teachers. The older children were observed playing, talking and laughing, watching and listening to a Smurf presentation, during quiet time and sleeping. Standard #: 22VAC40-185-270-A Description: Based on inspection the facility failed to repair a broken window in one of the classrooms. Evidence: 1. On 05/26/2017 the licensing inspector and staff #1 were inspecting the facility. There was a broken window in classroom #1. 2. On 05/26/2017 staff #1 confirmed that a neighborhood child had thrown a rock breaking the window on the outside pane. Plan of Correction: We will have this repaired immediately.
Standard #: 22VAC40-185-440-L Description: Based on observation there was a crescent neck pillow in the crib occupied by a four month old child. Evidence: 1. On 05/26/2017 the licensing inspector observed and inquired about the crescent neck pillow in the crib occupied by a four month old child. 2. On 05/26/2017 staff #1 confirmed the crescent neck pillow in the crib occupied by child #1 had been brought in by the child's mother. Plan of Correction: The pillow was immediately removed from the crib. A notice to the parent has been prepared that if the child needs the pillow then a parent and doctor's authorization is required.
Standard #: 22VAC40-191-40-D-1-C Description: Based on record review there was no updated criminal record check for one staff member. Evidence: 1. On 05/26/2017 the licensing inspector reviewed the record for staff #1 and there was no updated criminal record check. The most recent record check was dated 04/19/2014. 2. On 05/26/2017 staff #1 confirmed that the most recent criminal record check was 04/19/2014. Plan of Correction: I had sent the wrong form to the State Police who then sent it back along with other processed criminal record checks. I did not realize that I had made an error and needed to do anything further. I have gone on-line and obtained the SP167 which I will get notarized and in the mail to be processed on Tuesday. I should have this back within 7 - 10 days.
