63.2 Facilities and Programs..

Technical Assistance:
Children nine years of age and older may administer their own sunscreen if supervised. Sunscreen does not need to be kept locked but shall be inaccessible to children under five years of age or those children in a therapeutic child day program or special needs child day program

Comments:
An unannounced monitoring inspection was conducted this date. A total of 48 children present during the inspection with 4 staff present, within supervision guidelines. Observation of physical plant made. Daily health, CPR/First Aid, emergency contacts for staff and MAT training all verified on site. Staff records are reviewed annually in the administrative office. Snack today was juice and Cheez-Its. Three medications on site, all three reviewed and authorizations verified. A total of 10 children's records were reviewed. One violation found regarding recordation. Three children with food allergies, staff presented knowledgeable of their allergies and the course of treatment if exposed. The children transitioned from outdoor play to inside play in the gym and completing painted art projects that consisted of bobble head pets. Small groups of children observed playing legos and reading. Positive interaction observed between staff and children. Please call if there are any questions that may arise. Thank you for your cooperation today. The inspection occured between 4:15pm and 5:55pm.

Standard #: 22VAC40-185-60-A
Description: Based on record review, documentation of proof of identity was not complete. Evidence: Child #1,#2 and #4's record did not include the staff's name who viewed the proof of identity documentation.

Plan of Correction: Program will use the form that has the staff name for verification.
