22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, group time, activities, lunch, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-two (32) children were found to be in care with 4 staff, kitchen staff, and administrative staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 11:40am and concluded at 2pm.
Standard #: 22VAC40-185-330-B Description: Based on review of the playgrounds the facility failed to maintain compliance with this standard as required relating to resilient surfacing depth. Evidence: On the schooler playground under and around the swings and climbing structure there was not adequate resilient surfacing depth as required by ASTM guidelines. The depth measured approximately 2.5 inches. Plan of Correction: The mulch had just been "fluffed" the other day, but with the rain it has become compacted and dispersed again. Will we "re-fluff" the mulch again and order more if the required depth is not met at that time to ensure future compliance.
Standard #: 22VAC40-185-510-C Description: Based on review of medications and associated paperwork the facility failed to maintain compliance with this standard as required relating to administration permission. Evidence: The medications for child A did not have a long term use physicians permission to administer form filled out, signed, and dated as required. The parental permission to administer had expired, and had not been renewed every 10 days as required. The child started care with medications in April 2017, when permission was given to administer at that time. Medication has been administered daily to the child while in care, based on documentation reviewed and discussions with staff. Plan of Correction: Parental permission will be obtained again and renewed every 10 days as required until a physicians long term use permission to administer form can be obtained. In the future staff will verify all documentation required to administer medications is in place.
